Please make sure your student completes all forms listed below. No exceptions.

All Athletes:

  • EL2 (Sports Physical)
  • EL3 (Consent for Liability)
  • Heat Illness Prevention

    All Students need to Register with the NFHS and complete this course. They will need to print and turn in the certificate to the Athletics Office. 

  • Sudden Cardiac Arrest

    All Students need to Register with the NFHS and complete this course. They will need to print and turn in the certificate to the Athletics Office. 

  • Concussion Video

    All Students need to Register with the NFHS and complete this course. They will need to print and turn in the certificate to the Athletics Office.
