The list of immunizations below, a physical examination, and a copy of a birth certificate are required BEFORE a student may begin school.
Immunization: Florida law 232.032 requires that all students must provide the school with evidence that they have been immunized. It is important that specific dates be listed for each immunization.
The following immunizations should be completed for all students:

5 DPT (Diphtheria, Pertussis, Tetanus)
- 4 OPV (Oral Polio)
- 2 MMR (Measles, Mumps, Rubella)
- The Hepatitis B Vaccine should be on record for all students in grades K5 – 12. (Please note that the Hepatitis B Vaccine is a series of two or three shots that must be taken over a 3-6 month period.)
- Varicella (chickenpox) vaccine is required for all Elementary students and two doses are required for Kindergarten students.
- A Tetanus-Diphtheria Booster should be on record for all students in grades 7 – 12
Physical (Health) Examinations: The following students are required by Florida law to have a new physical:

All students transferring from another state or country

All students entering Kindergarten

All students entering 7th grade

All students participating in organized sports must have a sports physical. (This applies to team sports only).

The physical examination must have been completed within 12 months, prior to entering school.
